PCard Change Request

Use this form to make any of the following changes to an existing Purchasing Card (PCard):

  • Cancel a PCard
  • Increase Transaction or Monthly Limit
  • Address Change
  • Departmental Name Change
  • Departmental Card Coordinator (DCC) Name Change
  • Default Account Change

Send completed form to your Campus Card Coordinator with the following included in the subject line: PCARD CHANGE REQUEST.
